Position Summary

The Registered Nurse plans, organizes and directs Hospice care and is experienced in nursing, with an emphasis on community health education / experience. The Hospice nurse builds from the resources of the Interdisciplinary Group (IDG) to plan and direct services to meet the needs of individuals and families within their home environments. May provide patient care to premature infants, infants, pediatrics, adolescents, young, middle and older adults.

Position Specific Responsibilities

  • Completes assessment of patient and family to determine Hospice needs.
  • Provides a complete physical assessment and history of current and previous illness(es).
  • Provides professional nursing care by utilizing all elements of nursing process.
  • Assesses and evaluates patient’s status by:
  • Writing and initiating Plan of Care in Collaboration with the IDG.
  • Regularly reevaluating patient/family/caregiver needs.
  • Revising the Plan of Care as necessary in collaboration with the IDG.
  • Initiates the Plan of Care and makes necessary revisions as patient status and needs change in collaboration with the IDG.
  • Uses health assessment data to determine nursing diagnosis.
  • Develops a care plan in conjunction with the patient/family/caregiver and IDG which establishes goals based on nursing diagnosis and incorporates palliative nursing actions.
  • May administer medications and treatments as prescribed by the IDG approved Plan of Care.
  • Counsels the patient and family in meeting nursing and related care needs.
  • Provides health care instructions to the patient as appropriate per assessment and plan.
  • Fulfills the obligation of requested and/or accepted case assignments.
  • Participates in on-call duties as defined by the on-call procedures.
  • Maintains productivity standards as set by organization guidelines.
  • Documentation / Information Management:
  • Completes, maintains and submits accurate and relevant clinical notes regarding patient’s condition and care given.
  • Records pain/symptom management changes/outcomes as appropriate.
  • Communicates with the IDG regarding the patient’s needs and reports changes in the patient’s condition; obtains/receives physicians’ orders.
  • Communicates with Patient Care Manager to coordinate the care plan, when necessary.
  • Provides information and counseling to maintain a safe environment for the patient.
  • Assists the patient/family/caregiver and other team members in providing continuity of care.
  • Works in cooperation with the family/caregiver and Hospice IDG members to meet the emotional needs of the patient/family/caregiver.
  • Attends IDG team meetings to review and revise patient Plans of Care.
  • Submits documentation for the patient medical record within the timeframe set by the organization guidelines.
  • Leadership / Supervision:
  • Supervises ancillary personnel and delegates responsibilities when required.
  • Participates in orientation, education and development of team members as directed by supervisor.
  • Development:
  • Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and in-service classes.
  • Performance Improvement:
  • Actively participates in the Performance Improvement Program and participates in and adheres to the PH&S/TrinityCare Hospice Corporate Integrity Plan.
  • Participates in appropriate continuing education as may be requested and/or required by immediate supervisor.
  • Accepts personal responsibility for other educational activities to enhance job-related skills and abilities.
  • Budgetary:
  • Follows Core Value for being financially prudent.
  • Ensures that arrangements for medications covered in the Plan of Care, supplies, equipment and other necessary items and services defined in the Plan of Care are available.
  • Supervisory Authority: Makes recommendations regarding CHHA appraisal and assignment to the Patient Care Manager.

Education And/or Experience

Education Required - Nursing, graduate of a nursing school.

Experience - 2 years Nursing experience preferred. One year hospice, home health, skilled nursing facility, medical/surgical or oncology preferred.

Skills

  • Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
  • Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
  • Language Skills: Ability to understand, read, write, and speak English. Articulates and embraces integrated healthcare at home philosophy.
  • Computer Skills: Preferred working knowledge with computer applications, i.e.
  • Computer Skills: Microsoft Word/e-mail.
  • Other Skills and Abilities: Excellent observation, verbal and written communication, problem solving and basic math skills.
  • Other Skills and Abilities: Nursing skills per competency checklist.
  • Other Skills and Abilities: Visual acuity and hearing to perform required nursing skills.

Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. Adaptations can be made to accommodate team members with disabilities. Regular standing, walking, and manual dexterity are fundamental, along with the ability to lift and move objects up to 50 pounds. Visual acuity requirements include close and distance vision, color and peripheral vision, depth perception, and the ability to adjust focus. In a healthcare setting, exposure to bodily fluids, infectious diseases, and conditions typical to the field is expected. Routine use of standard medical equipment and tools associated with clinical care is essential. Compensation range: $54.51-$67.66 with bonus eligibility (10% short term incentive)

The amounts listed are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities
